- First Report of Injury or Illness (Form LWC-WC IA-1): This is the form that starts your workers’ compensation claim. Your employer is supposed to file it after you report your injury. But sometimes, you have to make sure your employer actually filed this report, or your claim won’t become active.
- Choice of Physician Form (Form WC 1121): Never forget that you have the right to choose your own doctor. Don’t select the first doctor your employer sends you to, because often this doctor cares more about your employer than you and your injuries. You use this form to make your choice.
- Notice of Payment (Form 1002): This form will show you how your weekly checks for lost wages are calculated. It’s important because the insurance companies don’t always get your amount right. And, when that happens, we can fight that.
- Disputed Claim for Compensation (Form 1008): When there’s a problem with your workers’ comp claim—like they’re denying your benefits or paying you less than you’re eligible for—this is the form you file to start fighting your employer in court.
- Disputed Claim for Medical Treatment (Form 1009): If the workers’ comp insurance company refuses to cover medical treatment that you need because of a job injury, this is the form you use to dispute that decision.
